SCAT Disability ID:  Download the application form (English or Spanish) in PDF format.  Bring the completed application form to the Customer Service Center.  Your photo will be taken and you will receive your ID card immediately.

75 PLUS ID:  Bring in identification with your date of birth showing that you are at least 75 years of age.  Your photo will be taken and you will receive your ID card immediately.
